DC2 Floating Support Service

DC2 is the floating resettlement support service for 10 young people aged 16-25 years old living in the community

As part of the development plan, consideration is given to housing options and the sort of housing young people would like to move to.

The scheme aims to provide young people with a place to live, resettlement support, the opportunity to build on the skills that have been learnt at Ty Bronna or Ty Danescourt, the opportunity for young people to reach their full potential for their long term independent living and to regain their self respect and self confidence.

Internal referral forms are completed for those young people living within Ty Bronna and Ty Danescourt who have developed their independent living skills and demonstrated their capabilities for moving to the third stage of accommodation and support towards full independent living.

Any young person from outside of Church Army Residential Services Cardiff fulfilling the eligibility criteria for the DC2 resettlement scheme can be referred via an external referral form which can be obtained from the Resettlement Support Officer at Ty Danescourt.

When a vacancy arises within the DC2 resettlement scheme the referral forms are checked and an assessment made of the young person most suitable to access the vacancy and a suitability needs assessment interview arranged. Following the suitability needs assessment interview, if the young person has been assessed as having needs suitable for floating support in the community, arrangements are made for them to move into the identified accommodation and for floating support to begin.
